Baseball, Softball, Boys Lacrosse, Girls Lacrosse, Boys & Girls Track and Field.  The start date for middle school spring sports is 3/10/24, and all MS registrations are due by 3/3/24.

In order to participate in spring sports, every student-athlete must complete the following three requirements:

  1. Register (1st time users) or log into your existing account on FamilyID (don’t use your student’s school email!)

Registration through FamilyID

Please follow the links below to complete your registration for your spring sport. Please login to your current account used for previous sports seasons OR create a new account only if you have never participated in OJR PIAA Athletics.  Please remember to use the same email address when accessing Family ID, as this will prevent FamilyID from creating multiple accounts for your student athlete.

  1. Submit a PIAA Section 6 CIPPE Physical Form for their 1st sport season of the school year (must be dated May 1st, 2024 or later). This only needs to be done once for the 2024-25 school year.

Completed PIAA Physical for 1st sport of the 2024-25 school year

Only a PIAA Physical will be accepted/approved for participation, any other form will not be accepted. The PIAA Section 6 form is located under “Forms/Info” at

https://bigteams-public-prod.s3.amazonaws.com/library/files/demo1221.bigteamsdemo_com_2083/files/PIAA%20section%206%20(dr%20physical%20form).pdf

Some doctors may require the PIAA Section 5 (health history) to complete the physical.  We do not need this form turned into us. The PIAA Section 5 is located under “Forms/Info” at

https://bigteams-public-prod.s3.amazonaws.com/library/files/demo1221.bigteamsdemo_com_2083/files/PIAA%20section%205%20(helath%20history).pdf

Print and take the Section 6 form to your physical (and the Section 5 form if your doctor requires it). Your physician must complete the entire PIAA Section 6 form, with a valid date and signature (if missing the form is invalid). If "other medical recommendations" are listed on the physical by the examining physician, please have them completed & include a doctor's clearance for that recommended evaluation/treatment. Scan and submit your completed Section 6 CIPPE form to ojratc@ojrsd.net or mail a hard copy to:

  1. Complete a current Impact Baseline Assessment if needed

            Baseline Impact Test Assessment:

If your Impact test is NOT up to date, you will receive an email from either Brandon Clayville, Brian Falencki, or Kristin Paisley with a private link to complete your baseline assessment virtually. All 7th, 9th, and 11th-grade student-athletes are required to have a new baseline assessment completed for their 1st sports season of the school year. If a current 8th, 10th or 12th-grade student did not participate in a sport last school year, they must also have a new baseline assessment completed. These communications will go out after you have completed your registration. Please note: athletes will do a baseline concussion test whether their sport is deemed contact or non-contact.

If you have questions or concerns please contact Brandon Clayville, Brian Falencki, and Kristin Paisley at ojratc@ojrsd.net or Diana DeJoseph at ddejoseph@ojrsd.net

Thanks!
